StreetWorks, a program of Lutheran Social Service of MN, is a collaborative of youth-serving nonprofits that have provided street-based outreach to youth experiencing homelessness in the Metro area since 1994. We are the largest outreach collaborative in Minnesota serving homeless youth.
Our collaborative model has proven to be an effective way to conduct coordinated outreach across a broad metro area. By combining our efforts, outreach workers provide young people with connections and needed resources such as housing, case management, counseling, life skills training, food, family reunification and safety planning, emergency shelter, and other basic necessities.
SWC provides a coordinated intervention to meet the immediate needs of runaway and homeless youth and to interrupt exploitation by providing valuable resources, connection to services, and regular interaction with a trustworthy adult.
The mission of the StreetWorks Collaborative is to foster a diverse collaboration of youth, communities, and youth-serving agencies that coordinate effective and impactful street-based, site-based, and school-based outreach programs.
Through partnering with member agencies and other youth-serving entities, StreetWorks strives to increase access to resources and opportunities for homeless and at-risk youth, and ensure that programs and services are based on best practices. Our primary purpose is to walk alongside young people who are often invisible in the system, meeting them where they are and acknowledging and honoring their individual journey.
As a collaborative, StreetWorks believes that youth are resilient, full of hope, dreams, and potential – having the ability to succeed in life despite their histories of abuse, exploitation, neglect, and/or abandonment.
