To make an appointment with a Coordinated Entry Assessor in your area find the Coordinated Entry Access Points below.
Youth ages 16-24
- Please visit launchministry.org and click “Get Help Now” to schedule a crisis intake.
- Or you can schedule an intake by emailing admin@launchministry.org
- You can also call (952) 236-7141 (calling receives the slowest response as messages are only checked once per day).
